If your insurance requires a referral, you are responsible for obtaining the referral and seeing it reaches our office before your appointment. We also ask that you have your physician send any documentation, test results, etc. that would help our physicians understand your problems and understand what they are to see you for on this visit. This will also eliminate the need for duplication of tests in order to help diagnose and treat your symptoms.
